Minimum of 2 years License: Required Certifications: ACLS, BLS,
NIHSS Must-Have: Progressive Care Certified Nurse for Charge Nurse
role Description: Stepdown nurses are required; not Med/Tele. This
unit is a true stepdown unit with telemetry and cardiac
intervention capabilities. There are 20 telemetry beds, 16 cardiac
intervention beds, and 8 overflow telemetry beds, all operating
under the same cost center. The nurse-patient ratio is 1:3 or 1:4
based on acuity. The electronic medical record system is Cerner.
Charge nurses will run the unit without patients. Nurse aides are
provided at a ratio of 1 per every 12 patients and increase to 3
with a full unit. Weekend requirements include every other weekend,
with shifts on Friday/Saturday nights or Saturday/Sunday days.
Holidays are required on a rotating basis every other holiday.
Scrub color is navy. Telemetry is managed with Phillips monitors,
and nurses must read and interpret strips. Monitor technicians are
available. There are no full ventilators on this unit; however,
CPAP, BiPAP, and high-flow oxygen are utilized. Hospitalists or
intensivists are present 24/7. An IV team is available from 08:00
to 16:00. Respiratory therapy is available 24/7. Pharmacy services
are available 24/7. Shifts include 7a-7p, 3p-3a, and 7p-7a.
Floating is not applicable for full ventilator care. Common
diagnoses include post-catheterization patients, both clean and
interventions, post-TEE, post-cardioversion, ablations, AICD
patients, and occasionally external temporary pacer patients.
Knowledge of chest tubes, tube feedings, and multiple drains is
essential. Nurses must be well-versed in titrating medications
including amiodarone, cardizem, nitroglycerin, pressors, dopamine,
insulin, heparin, and others. Stepdown nurses are specifically
