Patients and their families know us for our personalized, high quality care, delivered with empathy, kindness, and respect TITLE: Hospice Scheduler OFFICE LOCATION: Sunnyvale SCHEDULE: Full Time HOURS: 8:30am-5pm POSITION SUMMARY: The Hospice Scheduler is responsible for assuring the smooth operation of the scheduling support functions of the assigned Branch Office to assure optimal patient-centered care, efficient utilization of services, and compliance with ordered visit frequency. The Scheduler is responsible for scheduling operations, including coordinating/assigning visiting disciplines to patients and coordinating schedules among multiple providers and disciplines. AREAS OF RESPONSIBILITY: Coordinates scheduling of hospice patients with assistance of Patient Care Manager (PCM) and/or Resource Nurse. Responsible for maintaining the team master schedule and visit verification. Updates direct care staff schedules weekly and as needed. Performs visit verification activities daily. Verifies visits provided to visits ordered, scheduled, and documented. Notifies PCM of discrepancies between visits ordered, scheduled, and documented/verified. Coordinates with Intake to schedule all patient admissions Coordinates initial visits for internal multidisciplinary referrals as requested by PCM Works under the direction of the PCM or Resource Nurse to rearrange schedules in situations of call-offs, conflict or high utilization of service. Ensures prompt scheduling of emergent/as-needed visits as directed by the PCM or Resource Nurse. Collaborates with clinicians regarding scheduling issues, conflicts, or concerns. Assures timely completion and synchronization of individual staff schedules within established time parameters. Accesses patient and provider schedules, visit frequency, and physician orders to determine appropriate visit schedules under the direction of the PCM or Resource Nurse. Assures efficient use of staff by utilizing full-time staff prior to scheduling part-time and per diem staff in consultation with the PCM or Resource Nurse. Manage Master schedule and distribute Monthly and Vacation schedules under the supervision of the PCM or Resource Nurse. Create and maintain administrative resources to ensure efficiencies. Coordinates, assigns, and notifies appropriate on-call triage staff and management of Weekend/Holiday schedule. Answer and screen incoming telephone calls related to patient scheduling; directs all calls to the appropriate person or department as needed. Performs other duties as assigned consistent with skills and training and the mission and goals of Pathways. QUALIFICATIONS: Two years medical office administrative support experience. Scheduling experience. Knowledge of medical terminology. Computer proficiency; Microsoft Word, Excel and database management applications. Well organized, flexible, able to adapt rapidly to changing priorities and tasks; willing to learn new tasks. Ability to work professionally with people.
