Director of Rehab Location: Watsonville, CA Agency: Acruex Health Pay: $2,040 to $2,160 per week Shift Information: Days - 3 days x 10 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Pay Rate: $68.00 - $72.00/Hourly. Summary: Leads and manages all rehabilitation services (PT/OT) in the Skilled Nursing Facility. Ensures high-quality patient outcomes, regulatory compliance, efficient operations, and strong team performance under SNF guidelines and PDPM requirements. Responsibilities: Oversee daily PT/OT operations, scheduling, and caseload management. Ensure compliance with SNF regulations, PDPM coding, documentation standards, and medical necessity. Monitor therapy outcomes, patient progress, and safe discharge planning. Recruit, train, supervise, and evaluate rehab staff. Manage productivity, staffing levels, and department budget. Collaborate with Nursing, MDS, Physicians, and IDT to support care plans and optimize PDPM accuracy. Maintain quality, safety, and patient satisfaction standards. Requirements Requirements: Licensed PT or OT (state requirement). 2-3 years SNF or post?acute rehab experience; leadership experience required. Strong knowledge of PDPM, MDS interface, documentation, and reimbursement. Effective leadership, communication, and organizational skills.
