Assistant-LPN Overview: The Primary Care Assistant-LPN delivers
patient-focused care under the supervision of primary care
providers, prioritizing safe, timely, and effective clinical
assistance within a primary care environment. This role
necessitates a minimum of six months of clinical healthcare
experience and an active LPN license, highlighting the importance
of teamwork, communication, and the ability to manage tasks
independently. This position is dedicated to enhancing quality

operations team, the Primary Care Patient Care LPN collaborates
closely with our Primary Care Providers to ensure safe, timely,

Primary Care Patient Care LPN will work alongside patients,
providers, and other clinical team members to promote quality
outcomes for all individuals. Responsibilities will be carried out

Position Details: Experience: A minimum of 6 months of clinical
healthcare experience, ideally in a primary care setting. Required
Education: Completion of an accredited practical nursing program.
Licensure: Must hold an active LPN (Licensed Practical Nurse)
license in the state of practice. Additional Qualifications:
Ability to collaborate effectively as part of a team, with strong
written and verbal communication skills. A customer service
mindset, excellent organizational and interpersonal abilities, a
flexible and positive demeanor, and a keen attention to detail and
