Job Summary: We are seeking a skilled and compassionate Physical Therapist to deliver high-quality home health rehabilitation services throughout the San Mateo region. The Physical Therapist will evaluate and treat patients using medically prescribed therapy programs aimed at restoring function, reducing disability, and helping individuals reach their highest possible level of physical performance. Responsibilities include completing admission and evaluation visits, supervising Physical Therapy Assistants, documenting clinical findings, goals, and discharge plans, and communicating OASIS scoring within required timelines. The ideal candidate demonstrates strong clinical assessment skills, effective documentation practices, and the ability to work independently while providing exceptional patient-centered care. Territory Coverage: Atherton, Redwood City, Belmont, San Mateo, Burlingame, Millbrae, San Bruno, and surrounding neighboring cities as needed. Requirements: - Minimum 1 year of Home Health experience - Active state Physical Therapist license - BLS/BCLS certification - Proficiency with EMR systems (Homecare Homebase HCHB preferred) - Ability to work autonomously in a home health environment Job Details: - Shift: Days, 8:00 AM–5:30 PM (flexible based on patient availability) - Weekend Coverage: One weekend day per month - Holiday Coverage: Not required - Contract Length: 13 weeks - Bilingual: Not required - Specialty: Rehabilitation Therapy – Physical Therapist - Setting: Home Health Only applicants who meet the minimum qualifications and possess the required certifications are eligible to apply.
