nurse RN Care Manager for a travel nursing job in Sunnyvale,
California. Job Description & Requirements Specialty: Care Manager

per week Shift: 8 hours, days Employment Type: Travel RN – Case
Manager (Hospice) Shift: Days, 8:30 AM – 5:00 PM (Schedule can be
flexible based on patient availability) Weekends: One weekend
rotation Position Urgency: Normal Guaranteed Hours: 40 hours/week
Contract Length: 91 days (13 weeks) Billing String: Medefis
Position Summary The Hospice Nurse Case Manager is responsible for:
Admitting patients to hospice services. Assessing the patient’s and
family’s physical, psychosocial, environmental, safety, and
developmental needs. Developing and documenting individualized
plans of care. Providing nursing interventions and documenting
observations related to the patient’s condition and environment.
Coordinating care with a multidisciplinary team. Participating in
the hospice program’s Quality Assessment and Performance
Improvement (QAPI) program. Conducting follow-up visits as
required. Requirements Experience: Minimum of 1 year of hospice
nursing experience required. License/Certification: Current RN
license in the state of practice. Other: Reliable transportation
for travel within the assigned territory. Territory Coverage
Includes, but is not limited to: Atherton, Menlo Park, Palo Alto,
Mountain View, Sunnyvale, Santa Clara, San Jose, Saratoga, Los
Gatos, Campbell, Morgan Hill, Gilroy , and neighboring cities as
