Job Description We’re hiring a Family Nurse Practitioner (FNP)
to join a high-impact community health organization serving
patients across California’s Central Valley. This FQHC is known for

administrative staff. What You’ll Be Doing: - Deliver full-spectrum
primary care to patients of all ages. - Diagnose and treat acute
and chronic conditions; perform exams and order labs/imaging. -
Coordinate with care teams (nursing, case management, behavioral
health) to provide wraparound services. - Document efficiently in
NextGen EHR, with time allotted for timely charting. - Participate
in team huddles, provider meetings, and quality improvement
initiatives. Who We’re Looking For: - Licensed Family Nurse
Practitioner (FNP) in California with active DEA and BLS. - 2 years
of experience in outpatient or community health settings strongly
preferred. - Deep commitment to health equity and providing
high-quality care to underserved populations. - Confident,
communicative, and collaborative—you value teamwork and patient
relationships. - Bilingual in Spanish (or other prevalent languages
in the Central Valley) is a big plus. Bonus If You Have: -
Familiarity with NextGen or similar EHR platforms. - Experience in
chronic disease management, population health, or value-based care
models. - A calm, flexible approach to navigating high-volume,
