Experience:

--- 12 months of Case Management experience is preferred.

--- 3 months of experience with Homecare Homebase (HCHB) is preferred.

Requirements:

--- Candidates must have a California license in hand at the time of application.

--- COVID vaccination required for onboarding. Religious and medical declinations accepted.

--- COVID booster required for onboarding. Religious and medical declinations accepted.

Additional Information:

Guaranteed Hours: 40, 8:30am-5pm (Schedule can be flexible based on patient's availability.) Weekend: One weekend per month.

Requirements: One year of Home Health experience.
