We might be a perfect fit for you! Position Summary: Performs patient nutrition assessment. Designs, implements, and continuously monitors each patient's nutrition care plan. Serves as part of the patient care team and provides nutrition recommendations as needed. Provides nutrition education to patients and staff. Primary Responsibilities Perform initial patient nutrition assessment and order diet, tube feeding and/or supplements utilizing approved format and within specified time frames. Meet with patients at least one time weekly to provide nutrition counseling and education and to review compliance with nutrition recommendations. Monitor weight daily and make appropriate changes to meal plan and/or tube feeding/supplements to assure appropriate weight restoration/maintenance/loss. Prepare materials and educate patients, as required per program schedule and documenting per facility protocol. Create and implement staff training as requested to improve patient care. Represent The Meadows Ranch at The Meadows as requested to educate the family members and professionals. Review and monitor patient menus, diet orders and tube feeding/ supplements on an ongoing basis to ensure nutrition needs are met. Ensure all aspects of nutrition care are provided in compliance with regulatory and certification standards. Job Requirements:Education, Skills & Experience Requirements Bachelor's Degree in nutrition science, foods and nutrition, food service systems management, or a related area. Registered with the Academy of Nutrition and Dietetics or Commission on Dietetic Registration (CDR) One year experience in inpatient hospital nutrition services or behavioral health nutrition services is preferred. Ability to converse with others to give receive, and process information in English, ability to relay clear written messages; extensive use of telephone. Ability to hear alarms and pages. Ability to maintain confidentiality of information, multi-task, coordinate projects and record/transcribe general and specific meeting minutes. Ability to work independently and collaboratively. Communicate with other staff to relay concerns and observations involving patient safety. Occasional/limited exposure to: Infection and to the risk of bloodborne diseases; hazardous agents, body fluids and wastes; unpleasant patient or departmental elements; to potentially violent/aggressive patients/staff/visitors, critical incidents, and/or sentinel events. Occasionally needed to work irregular hours or additional hours. Ability to initiate action, take personal responsibility, suggest improvements, and solve problems within scope of job without being asked Ability to understand and comply with government, regulatory, and company rules. Ability to record, report, and maintain confidentiality of information and respond to requests for information.
