Clinical Education Manager - Home Health and Hospice Education Sunnyvale, California Description For over 40 years Pathways has been a Bay Area pioneer, leader and innovator in Hospice, Home Health, Palliative Care, and Care Management services. TITLE: Education Program Manager- Home Health LOCATION: Sunnyvale, Ca SCHEDULE: Full Time HOURS : 8:30am-5pm (Hours are flexible) POSITION SUMMARY: This role is responsible for the overall education and competency of new employees and current staff of the Home Health clinical team. Including maintenance of and executing the following programs; new employee orientation, annual competency review, field supervisory visits and preceptor training. The Education Program Manager will have supervisory responsibility over preceptors, new hires in training and department support staff. This role works closely with the Director of Patient Care Services and Director of Quality, Education and Compliance to implement education strategies and programs necessary to meet the quality and patient care satisfaction goals. Ensures that all training programs and educational offerings will reflect compliance with state and federal regulations and follow evidence based clinical practice. AREAS OF RESPONSIBILITY: Monitors new clinical staff during their orientation period, including making home evaluation visits, and documentation oversight. Coordinates and collaborates with the clinical preceptors to assure standardization in mentoring methodologies. Collaborates on the development of clinical pathways and integrates knowledge and practice patterns for successful chronic illness management. Assists the manager to develop improvement plans with measurable goals and timeframes for newly hired staff who are not meeting orientation objective and productivity targets. Develops, implements and continually evaluates orientation and training activities/programs for all Home Health staff; including required competency testing and mandatory in-service education. Effectively implements disease management treatment protocols and educates on program factors relevant to OASIS. Provides OASIS-C training Conducts annual educational needs assessment based on data outcomes. Functions as the clinical expert and resource for development, education, training and trouble-shooting of the home care-specific electronic health record system. Stays abreast of trends in adult learning and accreditation and regulatory requirements. Communicates with appropriate clinical managers regarding staff progress and any human resource or performance counseling issues. Maintains effective communication with clinical managers and direct care staff to allow the integration of educational services into multidisciplinary programs. Participates on the Policy and Procedure Committee QUALIFICATIONS: Monitors new clinical staff during their orientation period, including making home evaluation visits, and documentation oversight. Current licensure as RN in State of California Bachelor of Science in Nursing required; master's degree in nursing or relevant field preferred. Two years of Home Health experience required; supervisory and preceptor experience preferred. OASIS C Certification preferred. Knowledgeable in electronic record systems, specifically in home care-specific electronic health record systems. Apply
