– Remote Position (San Mateo/Santa Clara County)
Must live within a commutable distance for patient and clinic
visits as needed. Travel to centers and practice offices to visit
patients in person, while also interacting with the virtually.

About the Role
The Kidney Care Manager will be responsible for providing care and
case management for patients, and delivering clinical care to
chronic kidney disease and end-stage kidney disease patients. This
position will be working closely with complex renal patients in
their home, dialysis center, by phone, virtually, and
electronically as needed. This is a market-based position, kidney
care manager will work with patients and the care team through
in-person, virtual, telephonic and electronic means of
communications, care coordination and management.
Essential Functions:
Care for renal patients by providing telephonic, electronic,
telehealth, or in-person center care.
Conduct clinical assessments and wellness exams - in the dialysis
center, physician's office, or in the patient's home.
Be a dedicated kidney care manager to renal patients, monitor their
progress and stay in regular communication with each patient and
their caregiver to address health concerns and conditions.
Connect patients with different members of the care team and
provide care coordination with internal and external providers,
hospitals and ancillary services, SNFs, home health, palliative
care and hospice.
Coordinate care with home health, nephrologists, PCPs, specialists,
vascular access centers, medical and transplant centers.
Educate renal patients on chronic kidney disease, ESRD and
treatment options, ensure timely access placement and follow up,
and optimal transition to dialysis, transplant, or compassion
care.
Document comprehensive patient assessments and records, scheduling,
follow-up labs, screening, and appointments as required.
Monitor key performance indicators for patients, improve patient
outcomes and lower costs.
Provide case management for high-risk patients with multiple
comorbidities.
Accountable for outstanding customer service to all external and
internal customers.

Minimum Education & Experience:
Bachelor's in Nursing and a minimum of 5+ years of renal nursing
experience, preferably across multiple settings.
Knowledge of kidney disease and comorbidities
Ability to make patient health assessments, and develop care
management plans and health goals.
Active Registered CA RN License or Active CA Nurse Practitioner
license or licensed Clinical Nurse Specialist
Experience conducting patient visits in the home, in the
physician's office, and via telehealth
Any combination of education and experience that would likely
provide the required knowledge, skills, and abilities as well as
possession of any required licenses or certifications is
qualifying
Knowledge, Skills & Abilities:
Proficient with MS Outlook, Word, PowerPoint, and Excel.
Able to read, write, speak, understand and satisfactorily
communicate with others in English in person, virtually, over the
phone and via email. Bilingual in Spanish is preferred.
Access to a car, a California driver's license and car
insurance
Good verbal, written, virtual and in-person communication
skills
Ability to work independently, and competent in using medical
instruments for patient examinations.
Position will be primarily field-based in the Bay Area, California,
with requirement to attend periodic meetings and training.
